This is an innovative role supporting and developing social prescribing in the Mid Cornwall area. Specifically, it will involve supporting the social prescribing services at St. Austell Healthcare, enabling patients to manage their health and well-being and developing links to activities and opportunities in the local community.

The role will be based at St. Austell Healthcare and will involve working as part of a local integrated team and collaborating with partners, including other Link Workers, the health sector, the voluntary sector, and identified community champions.

The role will engage with relevant community activities and build a network of providers who will host clients as part of the social prescribing service. Whilst the role will not exclusively concern nature based and outdoor activities, it will have a focus on this area.

This post is subject to a satisfactory Disclosure and Barring check.

Travel is an essential part of the role, the post-holder will therefore be required to hold a valid driving license for the UK / travel independently to meet the requirements of the role. Funding has been secured from the Department of Health and Cornwall Council to support this post.
